Has a team ever screwed up the play when doing the victory formation in football?

When a team is going to win the game in football (college or pro) they set up in the victory formation and kneel the ball. They often times have one player stand about 10 yards behind the QB. Has it ever happened where a team messed this up and the player standing behind the QB had to do something?

1. don't remember the team,but I saw a game when the qb fumbled the ball and the team recovered and ran for a touchdown.
2. No one will believe it happened to a great team like them. But, several years ago the Browns QB did the kneel and knocked the ball loose with his knee before getting down. No touchdown by the other team, or recovery by the deep guy. Just a recovery by the other team.
